Sample policy for a fictional business
1. What we collect
Your name, phone number, email and address; your pets’ names, ages and medical records; and payment records. If you fill in the new-client form, we also keep the notes you write about what helps your pet stay calm.
2. How we use it
To book and run visits, to send the reminders and summaries you asked for, to request records from your previous clinic when you tick that box, and to bill you. We do not sell your information, and we do not use it for advertising.
3. Who sees your pet’s records
Our vets, technicians and front desk. We share records with another clinic or a specialist when you ask us to, and with public health offices where the law requires it, for example rabies vaccination certificates.
4. Texts and email
You choose text, email or both in the client portal. Reply STOP to any text to stop texts. The four seasonal emails are separate, and you can unsubscribe from any of them.
5. How long we keep it
Medical records for as long as New York State requires after your pet’s last visit. Payment records for seven years. Everything else until you ask us to delete it.
6. Your choices
You can ask for a copy of your records, ask us to correct them, or ask us to delete what the law lets us delete. Write to hello@lanternwood.example or ask at the front desk.
